Support Coordination

Here at Equip U we take the time to get to know your goals so we can offer you the support coordination services that will serve you the most. Our support coordinator professionals on our team pride themselves on serving you and your needs, and connecting you to the most appropriate support to work alongside you in the most effective way.

We support a variety of individuals who have a disability or face mental health challenges and need tailored support coordination in managing a complex range of needs including:

• Individualised supports in the home and in the community

• Capacity building

• Community inclusion services
